What Are The Stages Of Breast Cancer?

Like other countries, India stages breast cancer using the TNM system, which was created by the American Joint Committee on Cancer (AJCC). The stages of breast cancer play a crucial role in forecasting the expected course and outcome of the disease as well as choosing the best course of treatment. The TNM system evaluates three main factors: Tumor size (T), lymph node involvement (N), and the presence of metastasis (M). Based on these factors, breast cancer is classified into stages 0 through IV.


Stage 0: Carcinoma in Situ

Ductal Carcinoma in Situ (DCIS)

  • This type of cancer is non-invasive, meaning that the abnormal cells are contained inside the duct lining and have not transferred to other breast tissues.

Lobular Carcinoma in Situ (LCIS)

  • Although not considered a true cancer, LCIS is a marker indicating an increased risk of developing invasive breast cancer in either breast.


Stage I: Early-Stage Breast Cancer

Stage IA

  • The tumour has not grown outside of the breast and is no more than two centimetres.

Stage IB

  • The lymph nodes contain small clusters of cancer cells that are bigger than 0.2 millimetres but not larger than 2 millimetres, and either no tumour is found in the breast or the tumour is 2 centimetres or smaller.


Stage II: Early to Locally Advanced Breast Cancer

Stage IIA

  • One to three axillary lymph nodes (lymph nodes under the arm) or the lymph nodes close to the breastbone are determined to be cancerous, but there is no tumour in the breast.

  • The tumour has either grown to the axillary lymph nodes if it is 2 centimetres or less or is between 2 and 5 centimetres and has not.

Stage IIB

  • The tumour is either greater than 5 centimetres but has not yet migrated to the axillary lymph nodes, or it is between 2 and 5 centimetres and has spread to 1 to 3 axillary lymph nodes or lymph nodes close to the breastbone.


Stage III: Locally Advanced Breast Cancer

Stage IIIA

  • No tumour is found in the breast, or the tumour may be of any size. Small clusters of cancer cells are found in the axillary lymph nodes, or the tumour is greater than 5 centimetres.

  • 4 to 9 axillary lymph nodes or the lymph nodes near the breastbone are found to be cancerous. One to three axillary lymph nodes near the breastbone have been affected by cancer.

Stage IIIB

  • The tumour has apparently spread to the chest wall or skin and caused swelling or an ulcer.

  • It may have spread to 9 axillary lymph nodes or lymph nodes near the breastbone.

Stage IIIC

  • Ten or more axillary lymph nodes, lymph nodes near the collarbone, or lymph nodes near the breastbone have been affected by the cancer.


  • The tumour may or may not be discovered in the breast. 


Stage IV: Metastatic Breast Cancer

Stage IV

  • The cancer has progressed to other body parts, such as the bones, liver, lungs, or brain, in addition to the breast and neighbouring lymph nodes.

  • This stage is also known as advanced or metastatic breast cancer.
